Medicines Optimisation in Care Homes (MOCH) team

The Medicines Optimisation in Care Homes team is made up of clinical pharmacists and pharmacy technicians who are here to support care homes in all aspects of medicines management including:

• Medication review for residents
• Reducing medicines waste
• Supporting the use and implementation of Proxy ordering
• Providing medicines advice and guidance
• Ensuring safe administration and appropriate record keeping of medication  

The Cheshire Formulary website (www.cheshireformulary.nhs.uk) includes a designated Care Home support section which provides a library of useful resources on a range of topics. This is easily accessible from the home page under the ‘Care Homes’ tab. Our weekly medicines check-in email also periodically includes information which we hope you find useful.
