Mayfly Advance Care Planning and Communication Skills Course
Session Overview
- Are you required to have conversations with patients and those important to them about their care?
- Do you support patients and those important to them to start to think about their care as they become less well?
- Are you interested in learning more about Advance Care Planning?
- Do you want to gain more skills and knowledge around Advance Care Planning?
If so then this day is for you – The day is divided into 4 main areas:
- Think about it
- Talk about it
- Record it
- Share it
This will help you to have conversations which identify what is important to the patients you care for.
Learning Outcomes
By the end of the session, the participant will be able to:
- In partnership with others, review assessments to take account of changing needs, priorities and wishes, and ensure information about changes is properly shared
- Demonstrate awareness and understanding of Advance Care Planning, and the times at which it would be appropriate.
- Use effective, sensitive communication skills when having Advance Care Planning discussions as part of on-going assessment and intervention.
- Where appropriate, ensure that the wishes of the individual, as described in an Advance Care Planning statement, are shared (with permission) with other workers.
- When appropriate, know what the Advance Care Planning statement contains, and how this will impact upon a person’s care and support.
- Work sensitively with families and friends to support them as the person decides upon their preferences and wishes during the Advance Care Planning process.
