Are you involved in identifying and advance care planning for patients or residents that are palliative or nearing the end of life? If so, this short session will tell you all that you need to know about a new Cheshire & Merseyside framework that is being promoted across Cheshire East Place.

Come along to one of our 1 hour online lunchtime sessions to find out more about how the Electronic Personalised Care Plan can support you and your patients.

• The Cheshire & Merseyside Electronic Personalised Care Plan- where to find it and how it works including integration with Electronic Palliative Care Coordination Systems (EPaCCS)
