Cheshire EPaCCS Workshop – Using electronic records to enhance patient experience at the end of life
Session Overview
- Understand how EPaCCS works in Cheshire and how different roles contribute to the information within it to inform care coordination, decision making and the commissioning of services.

Undertake/contribute to multi-disciplinary electronic assessment and information sharing for people nearing the end of life (appropriate to your role)

In partnership with others, review end of life care electronic assessments to take account of changing needs, priorities and wishes, and ensure information about changes is properly shared.

Where appropriate, ensure that the wishes of the individual, as described in an Advance Care Planning statement, are shared electronically (with permission) with other workers.

When appropriate, know what the Advance Care Planning statement contains, and how this will impact upon a person’s care and support.
PLEASE NOTE: participants must complete the EPaCCS workbook before attending this session for the content to be relevant.
Learning Outcomes
