The End of Life Partnership (“We”) are committed to protecting and respecting your privacy.

The information on this page (in addition to our other policies and terms of use) sets out the basis on which any personal data we collect from you, or that you provide to us, will be processed by us.  Please read the following carefully to understand our views and practices regarding your personal data and how we will treat it. Information we collect from you

We will collect and process the following data about you:

  • Information you give us. This is information about you that you give us by filling in forms on our site or by corresponding with us by phone, e-mail or otherwise. The information you give us may include your name, address, e-mail address and phone number, Job Title or Organisation of Employment.
  • Information we collect about you. With regard to each of your visits to our site we will automatically collect the following information:

  • Information we receive from other sources. This is information we receive about you if you use any of the other websites we operate or the other services we provide. In this case we will have informed you when we collected that data if we intend to share that data internally and combine it with data collected on this site. We will also have told you for what purpose we will share and combine your data.

  • Next of Kin Name
  • Next of Kin Contact Details
  • Patient Diagnosis/Conditions
  • Details of ongoing treatment
  • Details of current care circumstances
  • Referrer Name
  • Referrer Contact Details
  • Referring Organisation Contact Details

Uses made of the information – ADST

Information collected about patients or organisation may be used in a number of ways to support delivery of our support services including but not limited to:

  • Respond to referrals from individuals and services caring for people with advanced dementia who request guidance or
  • Support clinical teams and care services to identify the possible causes of behavioural and psychological symptoms of dementia
  • Support clinical teams and care services to implement Best Interests Advance Care Plans
  • Deliver education and information to informal/family carers on the nature of advanced dementia and likely
  • Discuss Advance Care Planning for future care to enable you to have confident conversations with G.P and health
  • Sit with you to talk about the nature of advanced dementia and how dementia affects a person
  • Support you, clinical teams and care services to implement Best Interests decisions
  • Work with clinical teams and care services to understand the possible causes of a loved one’s behaviour

Disclosure of your information

You agree that we have the right to share your personal information with:

  • employees and members of our organisation who require access to the information to perform our services;
  • employees and members of your organisation who require access to the information to perform services related to your employment or any service in relation to our contract with yourselves or your employer;
  • analytics and search engine providers that assist us in the improvement and optimisation of our site;

We will disclose your personal information to third parties:

  • If we are under a duty to disclose or share your personal data in order to comply with any legal obligation, or in order to enforce or apply our terms of use and other agreements; or to protect the rights, property, or safety of The End of Life Partnership, our customers, or others. This includes exchanging information with other companies and organisations for the purposes of fraud protection and credit risk reduction.

Disclosure of your information – ADST

You agree that we have the right to share your personal information with:

  • employees and members of our organisation who require access to the information to perform our services;
  • employees and members of your organisation who require access to the information to perform services related to your referral or any service in relation to our contract with yourselves or your employer;

We will disclose your personal information to third parties:

  • If we are under a duty to disclose or share your personal data in order to comply with any legal obligation, or in order to enforce or apply our terms of use and other agreements; or to protect the rights, property, or safety of The End of Life Partnership, our customers, or others. This includes exchanging information with other companies and organisations for the purposes of fraud protection and credit risk reduction.
  • Also involved with the wider care of a patient or individual where appropriate

Where we store your personal data

By submitting your personal data, you agree to the transfer, storing or processing of that data. We will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy.

All information you provide to us is stored on secure servers.

Unfortunately, the transmission of information via the internet is not completely secure. Although we will do our best to protect your personal data, we cannot guarantee the security of your data transmitted to our site; any transmission is at your own risk. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorised access.

Your rights

You have the right to ask us not to process your personal data for marketing purposes. We will usually ask for your consent (before collecting your data) if we intend to use your data for such purposes or if we intend to disclose your information to any third party for such purposes.

We do not process or use patient information for any other purposes than those outlined in this document and any other uses would first be discussed with the individual in question or person with legal power to make decisions of this nature
