Come along to one of our 1 hour online lunchtime sessions to find out more about how the Electronic Personalised Care Plan can support you and your patients.

Session Overview

The session will include the following:

1. An overview of ICARE & Share- what it is and how it applies to your role
2. The Cheshire & Merseyside Electronic Personalised Care Plan- where to find it and how it works including integration with Electronic Palliative Care Coordination Systems (EPaCCS)
3. Where to get further help, support and guidance

The Cheshire & Merseyside approach to personalised care planning for palliative and end of life care is being rolled out across Cheshire East Place.

We are holding lunchtime sessions to support health and social care staff to understand what this means for their role and for end of life care patients and their families.

Regardless of whether you work in Primary Care, Community Care, Acute Hospitals, Hospices of Care Homes, this will impact you in some way.

Many of the components of the ICARE & Share Framework are not new, they include approaches that EOLP and partners have been teaching and advocating for many years. However, there are some ‘newbies’ to share with you such as the Cheshire & Merseyside Electronic Personalised Care Plan.

Learning Outcomes

In partnership with others, review end of life care electronic assessments to take account of changing needs, priorities and wishes, and ensure information about changes is properly shared.

Where appropriate, ensure that the wishes of the individual, as described in an Advance Care Planning statement, are shared electronically (with permission) with other workers.

When appropriate, know what the Advance Care Planning statement contains, and how this will impact upon a person’s care and support.

Identify specific clinical events which may occur within a patient’s illness trajectory and support anticipatory clinical management planning (ACMP)*
