Managing the Heart Failure Patient through Primary and Secondary Care
Session Overview
This day looks at how to manage a patient with heart failure through both Primary and Secondary Care.
We will look at understanding the diagnosis through to complications in both primary and secondary settings, including palliation and End of Life Care.
The day will include specialists in heart failure care as well as a patient’s story.
The day includes lunch and refreshments.
Suitable for Healthcare professionals involved in heart failure care across community and acute settings, including:
- Primary care teams (GPs, practice nurses, community pharmacists)
- Specialist and multidisciplinary teams (complex case practitioners, frailty teams, virtual wards, advanced clinical practitioners)
- Acute care staff (hospital medics, cardiac unit staff, hospital pharmacists)
- Allied health professionals (physiotherapists, occupational therapists)
