Are you involved in identifying and advance care planning for patients or residents that are palliative or nearing the end of life?
This 1-hour Lunch & Learn session will tell you everything you need to know about a new Cheshire & Merseyside framework being promoted across Cheshire.
Suitable for all staff involved in identifying and advance care planning for patients or residents who are palliative or nearing the end of life.
Learning Outcomes
- You will have an understanding of ICARE & Share – what it is and how it applies to your role.
- The Cheshire & Merseyside Electronic Personalised Care Plan – where to find it and how it works, including integration with Electronic Palliative Care Coordination Systems (EPaCCS)
- Where to get further help, support and guidance

- Length: 1 hour, Lunch & Learn session
- Suitable for: GPs, Qualified Nurses, Community Nurses and Care Home Nurses. All staff involved in identifying and advance care planning for patients or residents who are palliative or nearing the end of life.
- Delivered: Online. See Dates below for details of times.
- Funding: Fully funded for those who work in Cheshire East Place and Cheshire West Place.
