Developing a Palliative and End of Life Strategy for Cheshire & Merseyside
Following the publication of the Population Based Needs Assessment (PBNA), Cheshire and Merseyside ICB are beginning to work on a new five-year strategy for Palliative and End of Life Care (PEOLC).
This strategy aims to:
- Raise the profile of PEOLC across the system
- Strengthen early identification and personalised care planning
- Improve outcomes and experiences for patients, families and carers
- Reduce avoidable unplanned hospital activity
An Invitation
We invite you to join an engagement workshop exploring how we can improve Early Identification and Care Planning to support the development of a Palliative and End of Life Strategy for Cheshire & Merseyside. The C&M End of Life Dashboard clearly shows that overall performance is behind ambition for Early Identification and Care Planning.
The PBNA shows a complex picture
- around 10% of all GP practices perform at over 50%, but around 8% score less than 10% (for Early Identification),
- there are stark difference between patients with cancer or dementia, and those with conditions such as COPD or heart failure,
- as well as differences in neighbourhood and between sexes.
Workshop Overview
The workshops will explore issues such as
- What would improve Early Identification and Care Planning performance (to get to our 60% targets)?
- What are the inequalities and unwarranted variation implications and how do we overcome these?
- What is the Impact on wider system, and how do we reduce this?
