Palliative and End of Life are Experience Survey

NHS Cheshire and Merseyside have established a Palliative and End of Life Care Programme Board to oversee the delivery of a programme of work that will enable access to good quality and equitable end of life care across Cheshire and Merseyside.
On behalf of the PEOLC Programme Board, the End of Life Partnership is gathering feedback to better understand people’s experiences of end of life care across Cheshire and Merseyside. Your insights will help us improve the quality and compassion of care for individuals and families in the future.
This survey will be open from November 2025 until the end of January 2026.
Why are we doing this?
We want to learn what is working / has worked well and what could be improved in the care provided during the final stages of life. Your feedback will inform service improvements and help shape future policies and strategies.
Who is this survey for?
This survey is for anyone who has experienced end of life care for themselves or supported a loved one through it in the past two-years.
There are three surveys that all ask similar questions, these surveys are targeted at:
- People who are currently receiving end-of-life or palliative care
- Relatives and friends of someone who is currently receiving end-of-life or palliative care
- People who have been bereaved in the last 2 years
Your privacy matters
- Your responses will be confidential and will not identify you personally.
- Data will be stored securely and handled in line with GDPR and data protection regulations.
- Only the project team will have access to the anonymised data.
- Information will only be used to share your views on Palliative and End of Life Care in Cheshire and Merseyside
