The End of Life Partnership was formed of the following organisations in 2014

 

Cheshire Hospices Education was established in 1998, and works across West, Central and Eastern Cheshire and Beyond. CHE is a provider of palliative and End of Life education, facilitating best practice. it also has a smaller research function. 

The End of Life Care Service Model was established in 2010, and works across Central and Eastern Cheshire. The EoLCSM team facilitates best practice, service and organisational development across all care settings. 

The Cheshire Living Well, Dying Well Programme was established in 2011. Its aim is to improve health and wellbeing by supporting a change in public knowledge, attitude and behaviour towards death, dying and loss so that living well, ageing well and dying well becomes the norm.
