Developing an All About Me folder will enable you to write down and store important information about your life.

Keeping information in one place will make things easier for you and your family if you are unable to look after your affairs, express your interests and following your death. It is important that we do this because our plans and wishes are more likely to be carried out if we write things down.

Lasting Power of Attorney - Finance (PDF)

While an individual has the mental capacity, they can set up a Lasting Power of Attorney (LPA) to give someone the authority to make decisions on their behalf. This person is known as an attorney.

There are two types - A Property and Financial Affairs LPA covers decisions about property and money.

Lasting Power of Attorney - Health (PDF)

While an individual has the mental capacity, they can set up a Lasting Power of Attorney (LPA) to give someone the authority to make decisions on their behalf. This person is known as an attorney.

There are two types - A Personal Welfare LPA covers decisions about healthcare and personal welfare. This can include allowing someone else to make decisions on your behalf about life sustaining treatment.

 I’ve included a family tree, photographs when I was young and my favorite joke. I have also included a love letter sent to my future wife in 1958.

 

 When my Dad had to go to a nursing home, the My Wishes Folder helped the staff to appreciate him as a person and ensure that they never serve him sea food!

 

 I have a sticker on my fridge explaining where My Wishes Folder is kept.

 

 I really enjoyed producing my folder. It also made it easier to talk about what the future holds for all the family.

 

 When my circumstances changed recently, it was easy to update my folder to ensure it still reflected my wishes.
